Personal
Your own information, routines, and choices. You decide what leaves this space.
Everyday health, practical routines, safety awareness, and care coordination, connected through the information you choose to share.
Connected Care brings the useful details together: the medication information you entered, the goal you chose, a journey you shared, and the person who agreed to help. You stay in control of your information and your relationships.
See goals, observations, and useful patterns together. Your cards connect to the same personal experience across Today, Health, Safety, Connect, and Places.
Sharing with a friend, a group, and a Care Organization should not mean the same thing.
Your own information, routines, and choices. You decide what leaves this space.
Invite people individually. Share different cards with different people. Sharing in return is always a separate choice.
Create a group with a name of your own, like Bennett Family. Each member chooses their own cards and audience.
Joining is not the same as sharing. New members do not receive existing cards until the people sharing them review the expanded audience. Care Organization access follows its own authorization, assignment, and card-sharing rules.
Keep the same familiar navigation as your needs and connections grow.
See the routines, goals, and practical work that make up your day.
Manage dated routines, tasks, errands, and appointments.
Personal goals and the Dynamic Cards you place here keep useful context close.
An overdue item, a completed occurrence, and a rescheduled task keep their own dates.
Check-In belongs in Safety; urgent care headers can direct you there.
Personal information and supported phone or wearable observations become useful cards, not a blanket feed to everyone you know.
Activity, sleep, workouts, heart-rate readings, respiratory information, and source-provided concern or fall information where supported.
Create named Dynamic Cards with up to four distinct datasets. Choose a presentation, history window, goals, and where a card appears.
Connect supported sources through your primary phone. Choose collection separately from the recipients and scope of each shared card.
Health and event availability depends on the source, device, operating system, permissions, and age eligibility. Source-provided events are not diagnoses by Connected Care. Refreshing the Web view does not take a new measurement.
Write what matters for your visit. Keep topics private or explicitly share them with your Care Organization.
Discuss the client’s authorized information in the same familiar five areas, without private provider work controls on display.
Request practical help. Separate an accepted task, a discussed topic, and the actual visit outcome.
No. You choose cards and scope for a person, a Sharing Group, or a Care Organization. Each group member controls their own sharing. A role, relationship, or payment by itself does not grant health-information access.
Connected Care has Web, iOS, and Android experiences. Supported health sources connect through your primary phone using Apple Health / HealthKit or Health Connect. Dataset, background, and location availability depends on platform support and permission. Browser use requires connectivity.
No. Sponsorship concerns payment and access, not ownership of the beneficiary’s account. They continue to control their relationships, sharing, and privacy.

Connect individually and choose what each person receives. Incoming and outgoing sharing are independent, and responder participation is a separate agreement.
Create a group, invite people, and review your own shared cards and included members. Group membership is not ownership of someone else’s information.
