One plan for the ordinary things that matter.

Create routines, appointments, tasks, and errands. See dated occurrences and what still needs attention; complete, skip, reschedule, or cancel the specific item without rewriting its history.

Personal Goals

Targets that fit your rhythm.

Track progress in daily and weekly views. Choose daily workout minutes, a weekly target, or selected days. Intentional rest days stay distinct from missing records, and changed goals do not rewrite earlier comparisons.

Dynamic Cards

Your information, arranged your way.

Create as many named cards as you need, with up to four different datasets per card. Choose a compatible summary, trend, comparison, or event view and place a card in the areas where it is useful.

Health Profile & Medications

A useful reference, not another medical record system.

Keep self-reported conditions, allergies, mobility and communication preferences together. Manage current medication information, optional reminders, and dated confirmations as separate choices. An Emergency Health Summary presents the relevant information compactly.

Phone & Wearable Health Sources

Bring supported observations into context.

Connect supported Apple Health / HealthKit or Android Health Connect information through your primary phone. Explore activity, sleep, workouts, heart-rate readings, respiratory information, and source-provided concern or fall information where supported.

Freshness You Can Understand

Know when information was observed, not just opened.

See the source and timing behind your information. Source updates, behind-the-scenes synchronization, and refreshing a browser view are different events. Missing information remains missing rather than becoming a zero.

Check-In & Response Plan

A clear way to ask, answer, and stay aware.

Choose people for your response plan and confirm their participation. Record a voluntary Check-In or respond to a specific request in Safety. A requested Check-In stays pending until the appropriate response or cancellation.

Request Help

Make the request, and the response, explicit.

Ask your eligible responders for support. See who acknowledges the request and who resolves it. The owner can end the request independently; reading a notification does not record a response.

Safety Timer

A little structure for a personal activity.

Start a timer for an errand or activity. Expiry prompts the owner; an unclosed timer can notify the first currently eligible responder once after the defined follow-up interval.

Places, Journeys & Safe Arrival

Share the journey on your terms.

Save familiar places, set an expected arrival, and choose journey-status sharing or journey location. Confirm arrival yourself and keep any follow-up separate. General Current Location has its own sharing and collection choices.

Connections & Sharing Groups

Choose people individually, or connect around a group.

Invite a direct Connection or create a custom-named Sharing Group. Group invitations and admission establish membership; each person controls their own shared cards and included members.

Share From the Card

Control the audience where the information lives.

Use a card’s share control to choose Connections, Sharing Groups, or Care Organizations. Review necessary scope in the same flow, or manage sharing from the recipient’s side. Preview your choices and understand overlapping access routes.

Conversations & Notifications

Keep the conversation with its people and purpose.

Use direct, group, and organization care conversations with clear audiences and references to permitted information. Account and device notification preferences stay distinct from human care actions.

Provider Work & Visits

A caseload overview and a client-centered conversation.

Move through People, Queue, Visits, Tasks, and Messages. Propose or record agreed visits, including recurring visits. Select a client to review their authorized information, and use Review Together during the conversation.

Agendas & Practical Handoffs

Turn a conversation into clear follow-through.

Prepare private or explicitly shared visit topics. Ask a named person to help with a practical task and follow its decision, acceptance, and completion. Reassignment requires fresh acceptance.

Coordinator Work & Organization Admin

Clear responsibilities within one application.

Manage staffing coverage, client assignments, team invitations, roles, and organization settings. Organization Administrators handle capacity and membership; Staffing Coordinators coordinate eligible providers.

Simple View & Two Languages

Make the interface comfortable for you.

Choose Simple or Detailed presentation, larger text, and English or Spanish system language. Preferences belong to your account on the device; another person using the device retains their own choices.

Account Protection & Support

Protection with a practical way back in.

Manage devices and sessions, choose optional multi-factor authentication, and use verified recovery for your existing account. Find support, report abuse, block a personal connection, or request data export and account deletion.

Plans & Sponsorship

Support someone without owning their account.

Use a personal plan, sponsor another person’s eligible access, or manage organization client capacity. A payer’s billing responsibilities stay separate from the beneficiary’s information and sharing choices.

Connected Care supports everyday health information and care coordination. It does not provide diagnosis, treatment, continuous emergency monitoring, or guaranteed response.
